DE ACORDO COM O DSM-IV, sintomas prodrômicos estão frequentemente presentes antes da fase ativa (...) e são formas relativamente menos graves ou subthreshold dos sintomas positivos especificados no critério A (alucinações e delírios). Indivíduos podem expressar uma variedade de crenças incomuns ou estranhas que não tem proporções deliroides, podem ter experiencias perceptivas incomuns, seu discurso pode ser geralmente compreensível mas digressivo, vago ou por demais abstrato ou concreto e seu comportamento pode ser peculiar mas não grosseiramente desorganizado. Em adição a estes sintomas do tipo positivo (positive-like) sintomas negativos são particularmente comuns na fase prodromica e podem frequentemente ser bastante severos. Individuos que eram socialmente ativos podem se tornar abstinentes, eles perdem interesse em atividades anteriormente consideradas prazerosas

First, advocates of all three positions recognize that people who meet UHR/CHR/prodromal criteria are symptomatic and in need of clinical care.[7,9•,12•–14•] Second, it is acknowledged that they have a greatly increased risk of developing a psychotic disorder within a brief time frame.[2,4,14•] Third, that they need treatment for current problems and to prevent transition to psychotic disorder.[7,14•,15] Finally, proponents of the different positions also all recognize that there is potential for harm in including the Risk Syndrome in the DSM-5.[9•,12•,16•] The relative importance that they place on these issues is perhaps the source of their apparent polarized positions. The central issues will be considered.

"New YorkHigh-Risk Project (NYHRP), a longitudinal prospective study in which initially unaffected and untreated offspring of schizophrenic parents, parents with major affective disorders, and parents without such disorders were followed from mid-childhood
through mid-adulthood. Major goals of the NYHRP as a whole were to identify neurobiological and other types of deficits as likely predictors and to establish some of these deficits as endophenotypes indicating genetic liability. Se consideramos que a psicose é uma instância sindrômica duvidosa, e que a esquizofrenia é um construto cientificamente impossível, não podemos com certeza dizer que uma pessoa irá ficar psicótica, ou desenvolverá "esquizofrenia". Neste sentido, tampouco nos é claro um conceito como o de pródromos.


HIPOTESE: com os clientes do GIPSI, ao contrário de um transtorno esquizoafetivo, verificam-se longos períodos de transtorno afetivo subthreshold que chegam a um ponto de estranhamento e retraimento social que é psychosis subthreshold também. A intervenção precoce pode prevenir o degringolamento de maiores crinificações psicóticas, o que traz o transforno afetivo que está underlying os sintomas psicóticos. E eles ficam normais e entediados depois de pararem de ouvir as vozes - é aí que recebem alta. A dissertação da Wal indica incidência de comprometimento afetivo, mais do que transtorno psicótico, embora SINTOMATOLOGICAMENTE estas pessoas tenham sido identificadas como psicóticas. O que faz uma pessoa identificar a outra como psicótica? A gravidade do estranhamento, por mais subthreshold para a psiquiatria. A cronificação é um encaminhar-se para o threshold e para o diagnóstico. O paradigma internacional encaminha-se para uma classificação com thresholds mais baixos, para facilitar o diagnóstico e a farmacoterapia ("it may be justifiable to target these individuals for intensive monitoring of mental state and even low-dose neuroleptic medication or other biological and psychosocial treatments depending on clinical condition" (Yung et cols., ). Nevertheless, to target suffering individuals, who elaborate demand for clinic care, is to develop new labels and new stigmatization (you´re not
schizophrenic - YET). To recognize that prodroms are symptoms (high risk of psychosis) and demand for health care - even though prodromal phase may never evolve into psychosis (and even believing that psychosis is such a confusingly defined syndrom that can be abandoned as well as its psychiatric diagnosis, and understanding that traditional psychiatry knows nothing about human development of their own angst, which is part of themselves and although may request for health care, cannot be extinguished)

Consideramos que a esquizofrenia é uma construção do tratamento tradicional da psiquiatria, e o que existe é um sofrimento psíquico grave, verificável por sinais do tipo psicótico. Este sinais seriam caracterizados por:
-serem de difícil manejo (Costa, 2003)
-indicarem um estranhamento (?)


Costa 2003 fala que o sofrimento psíquico grave é "toda manifestação aguda da angústia humana (...) que não é - ou não tem sido - bem compreendida pelos demais. (...) Não se trata aqui de negar que esista esta diferença radical, "bizarra" para muitos, mas antes de tentar resgatar, pela crítica analítica da linguagem, o espaço necessário para que a diferença como tal se revele e pernameça passível de múltiplas abordagens. (...) não existe a esquizofrenia, o que nao sifinifica dizer que não existam as pessoas demonimadas de esquizofrênicas"

Então o que diferencia os CDI+quaseDEPI e os PTI=3 dos neuróticos chatos pra quem a gente dá alta do gipsi?

O Rorschach subsidiando os achados sobre loucura relacional~~ - que a Yung fala de vulnerabilidade ao estresse e dificuldades de coping - e não consegue relacionar com número de eventos estressantes (até porque pessoas em uhr não tem EVENTOS DE VIDA) - acho que é porque ela desconsidera o AMADURECIMENTO destas pessoas inseridas em seus contextos familiares. A experiência de perda e de estresse de uma pessoa edipiana é certamente diferente de uma pré-edipiana e é por ficarem na superfície do fenômeno que os comportamentalistas pecam - e não me seduzem.

Psicose (Campbell, 1986) é uma perturbação mental gave, retraidora afetiva e intelectualmente e regressiva.

DSM-IV INTRODUCTION OF SCHIZOPHRENIA AND OTHER PSYCHOTIC SPECTRUM DISORDERS
O DSM-IV agupa a esquizofrenia e transtornos do espectro psicótico, mas alerta que isto foi feito para "facilitar o diagnóstico diferencial dos transtornos que incluem sintomas psicóticos como aspecto proeminente de sua apresentação". No entanto, "sintomas psicóticos não são necessariamente considerados os aspectos centrais ou fundamentais destes transtornos, nem os transtornos nesta seção tem necessariamente uma etiologia em comum".

A definição mais estreita de psicótico é restrita a delírios ou alucinações proeminentes, com alucinações ocorrendo na ausência de insight sobre sua natureza patológica. Uma definição menos restritiva também inclui alucinações proeminentes que o indivíduo reconhece como experiências alucinatórias. Mais ampla é a definição que também inclui outros sintomas positivos da esquizofrenia (fala desorganizada, comportamento grosseiramente desorganizado ou catatônico). Diferentes destas definições embasadas em sintomas, definições utilizadas em classificações anteriores eram provavelmente muito inclusivas e enfocavam na severidade do comprometimento funcional. (...) Neste manual, o termo psycótico refere à presença de certos sintomas. No entanto, a constelação específica de sintomas a qual o termo se refere varia em algum grau atraves das categorias diagnósticas. Na ESQUIZOFRENIA, TRANSTORNO ESQUIZOFRENIFORME, TRANSTORNO ESQUIZOAFETIVO E BREVE TRANSTORNO PSICÓTICO, o termo psicótico se refere a delírios, qualquer alucinação proeminente, fala desorganizada ou comportamento desorganizado ou catatônico.
No TRANSTORNO PSICÓTICO DEVIDO A UMA CONDIÇÃO MÉDICA GERAL e no TRANSTORNO PSICÓTICO DEVIDO INDUZIDO POR SUBSTÂNCIA, psicótico refere a delírios e somente alucinações não acompanhadas por insight.
No TRANSTORNO DELIROIDE e no TRANSTORNO PSICÓTICO COMPARTILHADO, psicótico é equivalente a deliroide.

É fácil diagnosticar as pessoas pelo DSM-IV:
-esquizofrenia é um transtorno que dura ao menos 6 meses (critério C) e inclui ao menos 1 mês de sintomas ativos (>1 sintomas psicóticos (positivos ou negativos) ou delírio bizarro/alucinação auditiva - critério A). É disfuncional social ou ocupacionalmente (critério B) e não tem causação orgânica específica (critério E) ou é precedido por transtorno de humor (critério D). Constelação de sintomas e sinais associados com comprometimento funcional;
-transtorno esquizofreniforme é apresentação sintomática semelhante à esquizofrenia que dura de 1 a 6 meses e em que não há declínio no funcionamento
-transtorno esquizoafetivo é apresentação de episódio de transtorno de humor na fase ativa dos sintomas de esquizofrenia, precedido ou seguido por ao menos duas demanas de delírios ou alucinações sem sintomas de humor.
-transtorno deliroide é 1 mês de delírios não bizarros sem fase ativa da esquizofrenia (delírios não bizarros são algo plausíveis e compreensíveis, devido a experiências de vida do indivíduo, mas ainda assim são de difícil compreensão e não diminuem sua convicção a despeito de evidências contraditórias)
-transtorno psicótico breve dura mais do que 1 dia e tem remissão em 1 mês.
acho que a maior parte dos clientes do gipsi começaram com transtorno psicótico breve e evoluíram para transtorno esquizofreniforme, transtorno esquizoafetivo e esquizofrenia (e.g. JAL) ao alcançarem 6 meses de DUP.

IMPLICAÇÕES ÉTICAS DE PESQUISAR FALSOS POSITIVOS NA FASE PRODRÔMICA: avaliar e não oferecer tratamento, para verificar se vão evoluir ou não para psicose?

It has not been used traditional psychosis scales (PANSS...), since PTI had been validated as sensitive and specific for psychosis spectrum disorders.

Estudo sobre pródromos

Estudos sobre o delineamento da pesquisa enfocam a importância de definir com rigor as questões iniciais - a partir delas, serão delineadas todas as etapas posteriores (método de coleta ou construção de dados, de análise, perspectiva da conclusão etc). Por mais que minha querida supervisora tenha dito que estas são questões que seguirão por toda uma carreira científica, eu gostaria de fortalecer o máximo possível minhas considerações teóricas, minhas escolhas.
Uma das questões importantes é o objetivo da pesquisa e os construtos a serem investigados. Estou questionando a descrição operacional de pródromos, não sei se gosto deste conceito.

"CHR for psychosis: recent-onset functional decline plus genetic risk, recent-onset subthreshold, or brief-threshold psychotic symptoms (Yung & McGorry, 1996).
The reliability of these criteria has been excellent, and studies using these criteria support the view that prodromal people are symptomatic and at high and imminent risk for psychosis (Cannon et al., 2008)".

Psychiatists... It´s likely that psychiatrists from Australia, Canada, Switzerland - all international great centers for "first psychotic-like crises" (if such a term applies for them) are from cognitive-approach-psichiatrists.

Abstract
The identification of individuals at high risk of developing a psychotic disorder has long been a goal of clinicians because it is thought that early treatment of this group may prevent onset of the disorder. However, little is known of predictive factors of psychosis, even within a high-risk group. This study followed up 104 young people thought to be at ‘ultra high risk’ for schizophrenia and other psychotic disorders by virtue of having a family history of psychotic disorder combined with some functional decline or the presence of subthreshold or self-limiting psychotic symptoms. All subjects were therefore symptomatic, but not psychotic, at intake. Thirty-six subjects (34.6%) developed frank psychotic symptoms within 12 months.
Measures of symptom duration, functioning, disability and psychopathology were made at intake, 6 and 12 months. Poor functioning, long duration of symptoms, high levels of depression and reduced attention were all predictors of psychosis. A combination of family history of psychosis, a recent significant decrease in functioning and recent experience of subthreshold psychotic symptoms was also predictive of psychosis. Combining highly predictive variables yielded a method of psychosis prediction at 12 months with good positive predictive value (80.8%), negative predictive value (81.8%) and specificity (92.6%) and moderate sensitivity (60.0%).
Within our symptomatic high-risk group, therefore, it appears possible to identify those individuals who are at particularly high risk of developing a psychotic disorder such as schizophrenia. Given the very high PPV and low false positive rate with this two-step process, it may be justifiable to target these individuals for intensive monitoring of mental state and even low-dose neuroleptic medication or other biological and psychosocial treatments depending on clinical condition. This indicated prevention approach could be further developed and preventive strategies in the psychoses refined.

Psychosis prediction: 12-month follow up of a high-risk (“prodromal”) group

Abstract
Intervention in the prodromal phase of schizophrenia and related psychoses may result in attenuation, delay or even prevention of the onset of psychosis in some individuals. However, a “prodrome” is difficult to recognise prospectively because of its nonspecific symptoms.
This study set out to recruit and follow up subjects at high risk of transition to psychosis with the aim of examining the predictive power for psychosis onset of certain mental state and illness variables.
Symptomatic individuals with either a family history of psychotic disorder, schizotypal personality disorder, subthreshold psychotic symptoms or brief transient psychotic symptoms were assessed and followed up monthly for 12 months or until psychosis onset.
Twenty of 49 subjects (40.8%) developed a psychotic disorder within 12 months. Some highly significant predictors of psychosis were found: long duration of prodromal symptoms, poor functioning at intake, low-grade psychotic symptoms, depression and disorganization. Combining some predictive variables yielded a strategy for psychosis prediction with good sensitivity (86%), specificity (91%) positive predictive value (80%) and negative predictive value (94%) within 6 months.
This study illustrates that it is possible to recruit and follow up individuals at ultra high risk of developing psychosis within a relatively brief follow-up period. Despite low numbers some highly significant predictors of psychosis were found. The findings support the development of more specific preventive strategies targeting the prodromal phase for some individuals at ultra high risk of schizophrenia.
